Customer Look-Up

The Customer Lookup page requires that you select a second personal identifier. The second identifier must match the date of birth to locate the customer's online profile.

Second Personal Identifier

You will need to select one of the following second personal identifiers:
  • Social Security Number
  • Customer ID
  • Other Document Type

To select the second personal identifier, follow the instruction below:

  • Select the second personal identifier from the Step 2 - Provide a Second Personal Identifier drop down list.
